Summary

The authors analysed rapid genome sequencing results obtained between 2019 and 2023 in hospitalized children, assessing management changes within 30 days of testing through chart review. Rapid genome sequencing was performed in 422 individuals, 339 in the intensive care unit and 83 outside it, with a diagnostic rate of 39% (32/83) in non-ICU versus 35% (120/339) in ICU patients. Among non-ICU diagnostic cases, 81% had a management change within 30 days and 56% (18/32) received a disease-targeted intervention, including medication or diet change, transplant listing, or referral to a clinical trial. Among these 18 children, the commonest disease categories were metabolic (61%, 11/18) and epilepsy (22%, 4/18).

Analysis

The useful finding is that the diagnostic yield outside intensive care is not lower than within it, which removes the main argument behind the reasoning still common among payors: restricting rapid sequencing to the sickest children. The methodological caveat is substantial: 83 patients, a retrospective single-centre series, and above all clinician-driven test indication, which enriches for strongly suggestive presentations and mechanically inflates yield. The practical lesson is the same as in intensive care: it is the therapeutic window — metabolic, epileptic, transplantation — that justifies urgency, not the ward the child is in.
