726 consecutive patients treated for early TNBC with neoadjuvant pembrolizumab + chemotherapy in the multicenter Neo-Real/GBECAM-0123 cohort are analyzed by germline BRCA1/2 status. Carriers of germline BRCA1/2 pathogenic variants show higher pathological complete response (pCR) rates than non-carriers, confirming in real-world data the synergistic effect between germline homologous recombination deficiency and immunotherapy. These data guide preferential pembrolizumab indication in germline BRCA TNBC.
