Neoadjuvant pembrolizumab plus chemotherapy in germline BRCA-mutated early triple-negative breast cancer: real-world multicenter data
Gene / mechanism
726 early TNBC patients (Neo-Real cohort), neoadjuvant pembrolizumab + chemo, analysis by germline BRCA status
Summary
726 consecutive patients treated for early TNBC with neoadjuvant pembrolizumab + chemotherapy in the multicenter Neo-Real/GBECAM-0123 cohort are analyzed by germline BRCA1/2 status. Carriers of germline BRCA1/2 pathogenic variants show higher pathological complete response (pCR) rates than non-carriers, confirming in real-world data the synergistic effect between germline homologous recombination deficiency and immunotherapy. These data guide preferential pembrolizumab indication in germline BRCA TNBC.

Analysis
Real-world data on pembrolizumab + chemo in germline BRCA TNBC are important because randomized trials (KEYNOTE-522) reported overall results without systematic stratification by germline status. This work partially fills this gap and reinforces patient selection of gBRCA for neoadjuvant pembrolizumab protocols.
