Swedish national cohort (1,208 pvBRCA1/2 women, 1994–2019): 507 risk-reducing mastectomies (RRM). Analysis of complications and occult cancers.

Summary

Swedish national cohort of 1,208 pvBRCA1/2 women between 1994 and 2019. Among them, 507 underwent risk-reducing mastectomy (RRM). Techniques: simple mastectomy (58.4%), nipple-sparing (28.2%), implant-based reconstruction (75.3%). 17/507 women (3.4%) had an occult cancer discovered at surgery. Major complications (reoperation) occurred in 3.7% of cases.

Analysis

Reassuring data on surgical safety of RRM for pre-operative counseling: major complication rate of 3.7% and occult cancer rate of 3.4% provide a solid evidence base. The 3.4% occult cancer rate argues for systematic histological examination of specimens. These Swedish data inform recommendations on surgical technique.
