CYP2D6-Guided Opioid Therapy for Chronic Pain: A Multi-Center Pragmatic Randomized Controlled Trial.
Gene–drug pair / mechanism
CYP2D6
CYP2D6-mediated metabolism of tramadol, hydrocodone and codeine into more active opioid receptor agonists
Summary
This pragmatic multi-center randomized controlled trial tested whether CYP2D6-guided opioid prescribing improves chronic pain. Adults with at least 3 months of chronic pain treated or considered for tramadol, hydrocodone or codeine were randomized to usual care or to a CYP2D6-guided arm, where prescribers were advised to avoid these three opioids in intermediate, poor or ultra-rapid (for safety) metabolizers, based on genotype or drug interactions. Of 1,048 enrolled participants, 282 had an actionable CYP2D6 phenotype. In this group, the primary outcome — change in composite pain intensity score from baseline to 3 months — was not improved by guided recommendations (mean [SD] -0.8 [2.4] vs. -0.7 [2.3]), nor was drug-gene concordance (26.8% vs. 27.1%). In an exploratory analysis, participants whose therapy was actually changed to align with their CYP2D6 phenotype had significantly improved pain scores.

Analysis
A negative trial on its primary outcome, and the authors read it accordingly: the EHR-integrated CYP2D6 alert did not change prescribing, with drug-gene concordance identical in both arms. This is an implementation failure rather than a refutation of tramadol and codeine pharmacogenetics, as the exploratory signal in patients whose therapy actually changed suggests — but that subgroup is self-selected and proves nothing. The practical lesson is that making a genotype available is not the same as changing practice: without prescriber support, the information goes unused.
